  2. Fill Item No. I (Name of the applicant)
In Item No. I (Name of the applicant), write your details, including your Aadhaar number.
    Open this page
  3. Tick Item No. II (Option for application)
In Item No. II (Option for application), tick Replacement voter ID card without correction, which means a replacement voter ID card with no changes.
  4. Attach a copy of FIR/Police report
Your voter ID card is lost, so attach a copy of FIR/Police report to the form.
  5. Sign the Declaration
Sign the Declaration to say your details are true to the best of your knowledge.
  6. Hand in Form 8 to your ERO
Give the filled Form 8 to your Electoral Registration Officer (ERO).

  7. After the last step
The ERO looks at your application.
Only people already on the voter list can ask for a new voter ID card on Form 8.
If your card is lost, the copy of FIR/Police report goes in with the form.
If your card is only damaged, you hand back the old card instead of a copy of FIR/Police report.
The guidelines page does not say how long this takes or how to track it.
